We perform comprehensive assessments which include the client's medical history, an assessment of cognitive abilities, functional and emotional status, assessment of strengths, living environment, finances, and existing social support.
From our assessments, we develop a care plan for short-term and long-term goals. The Care Plan is tailored to include recommendations regarding medical and health care issues, community resources, housing options, financial and legal planning.
Our Care managers monitor the progress and milestones of the plan to ensure that it continues to meet the person’s goals. Care Plans are modified as necessary to meet those goals. Care managers are available 24/7 to assist in emergencies and discuss concerns. We monitor and evaluate the overall health and support services provided to our clients.
We provide coordination of services when multiple disciplines are involved. Coordination of services may include home health agencies, homemaker and companion services, adult day care, special transportation or assistive technology.
