From our assessments, we develop a care plan for short-term and long-term goals. The Care Plan is tailored to include recommendations regarding medical and health care issues, community resources, housing options, financial and legal planning.
Our Care managers monitor the progress and milestones of the plan to ensure that it continues to meet the person’s goals. Care Plans are modified as necessary to meet those goals.
